TROPONIN I, BLOOD

Troponin is a thin filament regulatory protein with a unique cardiac form. Troponin I increases in blood about 4 hours after myocardial infarction and remains elevated about 8 days. It is recommended that troponin I be ordered when perioperative myocardial infarction is suspected and when myocardial infarction is suspected but MB-CK specificity is in doubt. Timing of specimens for this purpose should be 3 specimens drawn 8 hours apart.

Specimen Required:

 

 

Collect 3.0 mL of whole blood in a green-top (lithium heparin), plain red-top tube(s), or serum gel tube(s).

 

Laboratory Notes:

 

 

Spin, separate and send 1.0 mL (0.5 mL minimum) plasma or serum.

 

Reference Values:

 

 

Normal:

<0.07 ng/mL

Negative

 

 

Elevated:

0.07-0.24 ng/mL

May be consistent with myocardial injury/ischemia but is non-diagnostic and of equivocal significance. Consider obtaining additional Troponin values.

 

 

Positive:

³0.25 ng/mL

Positive Tropinin, suggest establishing rising or falling pattern and evidence of cardiac ischemia for consideration of myocardial infarction.

A lipid panel will be automatically performed for all patients with a cardiac troponin I value greater than or equal to 0.25 ng/mL. Recommendation of the National Cholesterol Education Panel.
