	Specimen Required:
	SUBMIT ONLY 1 OF THE FOLLOWING SPECIMENS:
		NOTE:		1.	Specify all antimicrobials being used to treat the patient.
				2.	Indicate PEAK or Trough. Specimens collected just before or within 15 minutes of the next dose represent the TROUGH levels. Specimens obtained within 15-30 minutes after the end of I.V. infusion or 45-60 minutes after an IM injection or 90 minutes after oral intake represent the PEAK level.
		Serum
		Draw 2.0 mL whole blood in a plain, red-top tube(s). (SERUM GEL TUBE IS NOT ACCEPTABLE.)
		NOTE:		1.	Indicate PEAK or TROUGH on request form.
	Laboratory Notes:
		Spin, separate and send 1.0 mL (0.5 mL minimum) of serum frozen in a plastic vial.

			Peak serum:			PO: 2.5 mcg.mL	IV: 10 mcg/mL
