	Specimen Required:
		BOTH EDTA AND SODIUM HEPARIN SAMPLES ARE REQUIRED!
		Draw 4.0 ml (2.0 ml minimum) whole blood in a purple-top (EDTA) tube(s) AND 4.0 ml (2.0 ml minimum) whole blood in a dark-green (sodium heparin) tube(s). NO GEL TUBES WILL BE ACCEPTED. COLLECT MONDAY – THURSDAY ONLY! Must arrive in lab by 1630 (Monday – Thursday ONLY!) Please send a complete Baylor Requisition & Consent Form with the samples.
	Laboratory Notes:
		DO NOT SPIN. Send BOTH 4.0 ml (2.0 ml minimum) EDTA tube AND 4.0 ml (2.0 ml minimum) Sodium Heparin tube at ambient temperature MONDAY – THURSDAY ONLY! Must have completed BCM GENETICS requisition sent with samples.
		NOTE:	1.	Orderable test name “Chrom Microarray” is for the Baylor test code #8655 (105K CMA).
			2.	If Athena test code #8635 (44K CMA) is checked on requisition, check with ordering physician to make certain that correct test is marked on req.
				a.	44K CMA tests will need to be ordered as ‘Misc Ref SLC’
			3.	Samples must be received at Baylor within 24 hours of collection. If received on Friday – Sunday, specimen needs to be cancelled and recollected Monday – Thursday.
